Homeless Outreach Substance Abuse Counselor - (Regular Full-Time)- Under the general supervision of the Executive Director and BHS Program Manager, assists in the delivery of behavioral health services by providing substance abuse counseling to individuals, couples, families and groups experiencing homelessness. The Substance Abuse Clinician will work as part of the agency's homeless team and will be responsible for conducting effective community outreach and counseling services, assisting with establishing a homeless collaborative aimed at eliminating disparities by addressing cultural barriers to services and for building community capacity in serving the homeless American Indians, participate in a network of support services to assist homeless individuals, perform required documentation for all encounters to fullfil reporting requirements. All My Relations Case Manager - (Regular Full-Time) - Under the general supervision of the All My Relations Project Manager, The Case Manager is responsible for conducting case coordination and management to Native American youth and families. The Case Manager is responsible for completig necessary documentation, attending meetings, writing up case management reports, provide translation services as requested, working with peacemakers, and traditional healers, assisting in preparations for the Advisory Committee meetings, recruiting Advisory Committee members and program participants, conducting follow-up, assisting and ensuring referrals for services to families are followed through in a timely manner. The Case Manager is also responsible for assisting in collecting data as assigned by the Project Manager. The Case Manager works collaboratively with all programs at FNCH, community members, and outside agencies. Job Description

Dentist - (Regular Full-Time) - Under the direction of Dental Program Manager the full-time exempt position Dentist is responsible for providing appropriate oral health information to individuals and groups and the direct provision of dental preventive services to patients of FNCH consistent with current dental protocols. Nurse Practitioner - (Regular Full-Time) - Under the direction of the Executive Director and Medical Director, the Nurse Practitioner shall work within the nursing process to provide direct nursing support to patients of FNCH. The Nurse Practitioner shall provide safe, direct patient care utilizing the nursing process in the ambulatory setting. The Nurse Practitioner shall provide clinical leadership for Primary Care staff and assist with other Primary Care duties as needed. The Nurse Practitioner shall serve as a positive role model to promote a positive work environment and provide quality patient care.
