HIV/AIDS Prevention and Case Management

First Nations Community HealthSource offers a full continuum of HIV care servicesPrevention icon that include harm reduction, overdose prevention education,  Naloxone distribution, confidential HIV testing and counseling, confidential Hepatitis C testing and counseling, HIV prevention education groups, PrEP, HIV treatment, Hepatitis C treatment and HIV medical and non medical case management services.

The following services are provided free of charge:

  • HIV/STD/Hepatitis education
  • Workshops for Native Women
  • Workshops for People Living with HIV/AIDS
  • Referral to Syringe Exchange
  • Free Rapid HIV testing and Counseling
  • Workshops for Native Gay Men
  • Workshops for Native Transgender Women
  • In House Referrals

First Nations Community HealthSource’ HIV Prevention and Case Management Services also operates the HIV Services Program (HSP) which provides the following medical and non medical case management services statewide for individuals who are living with HIV/AIDS:

  • Medical and Non-Medical Case Management
  • Transportation Reimbursement
  • In House Referrals
  • Client Advocacy
  • Substance Abuse and Mental Health Counseling
  • Emergency Rental/Utilities Assistance
  • Clinical Care
  • Referral to Work Re-Entry Assistance
  • Nutritional Information
  • Coordination of HIV and other Care Services

In partnership with the New Mexico Community AIDS Partnership, First Nations Community HealthSource operates Comida Buena, a food pantry for individuals who are living with HIV/AIDS in Albuquerque.
