Social Services Program
First Nations Community HealthSource’s Social Services Program provides case management and patient navigation services to assist with housing, vocational, recreational, subsistence, transportation needs, childcare, and other healthy lifestyle needs by providing referral information and program assistance. This program is comprised of FNCH’s Pathways, Intensive Case Management, Housing, basic subsistence assistance, and healthy lifestyle education. These programs are staffed by Community Health Workers, Patient Navigators, and Case Managers. These programs work in collaboration with community partners such as the University of New Mexico (Pathways), Presbyterian Health Services, New Mexico Coalition for Supportive Housing, International District Coalition, All Nations Wellness and Healing Center, Bernalillo County, and other organizations to provide support services as well as education about healthy nutrition, healthy cooking demonstrations, and community gardening.
