Free COVID-19 Testing Registration Form

First Nations Community HealthSource is offering free COVID-19 testing, Monday – Friday 8:30am – 4:00pm at the 5608 Zuni SE Clinic (West parking lot). You do not need to be a patient of First Nations Community HealthSource, have insurance or an ID, or be experiencing symptoms.

If you are interested in being tested, please complete the registration form below for each person who will be tested and indicate the date when you would like to be tested.
