Vision Statement

To eliminate homelessness and empower individuals and families to improve their health outcomes. The First Nations Community Healthsource (FNCH) Homeless Outreach Program (HOP) assists individuals and families who are homeless or at-risk of being homeless with healthcare and social support assistance by providing walk-in case management, and outreach services.

All individuals and families must complete the following to be eligible for HOP walk-in and case management services.

  • Register as a FNCH patient,

Job Development

FNCH Job Development offers FREE walk-in job training and computer lab services for community members. FNCH Job Development focuses on career planning and employment readiness.

Indigenous Plate Program

Serves warm meals to individuals who are experience hunger due to having limited financial resources.

Homeless Advisory Council

The Homeless Advisory Council (HAC) discusses issues and topics that affect the homeless

community and provides an opportunity for community members to provide feedback on FNCH and HOP services.

Art Group

The HOP Art Group is open to HOP clients and provides an opportunity to use the creative process of art making to improve and enhance the physical, mental, and emotional well-being of individuals.

Healthy Living Education Group

The HOP Healthy Living Education Series is open to all community members and   provides fun interactive activities for all ages, information on various health prevention and intervention topics, and dates for upcoming community events.

Street Outreach

HOP staff members seek out and engage with possible homeless individuals on the street, campsites, abandoned buildings, etc to promote FNCH services, provide health education      material, offer on-site Medicaid enrollment, and encourage individuals to become a patient of FNCH. HOP staff are out of the office for street outreach at least three times a week.

Am I eligible for HOP services?

Individuals and families are eligible for HOP services if they are in any of the following situations:

  • lack a fixed, regular, and adequate nighttime residence.
  • will imminently lose their primary nighttime residence.
  • fleeing, or attempting to flee, domestic violence or other life-threatening conditions that relate to violence.
What does establishing with a FNCH primary care provider mean?

Establishing a primary care provider means a client will see a FNCH medical provider a minimum of 2 times every 12 months. This medical provider will be the client’s first point of contact for all health issues. The goal is for the client to build a comfortable relationship with their provider, have confidence in asking questions, and receive whole-person care.

What is a HOP Intake?

During a HOP intake, a HOP Case Manager will complete an assessment with the client to identify any medical and/or social support needs. In consult with the client, the HOP Case Manager will prioritize long term goals, create a care plan, make appropriate referrals, assist with scheduling appointments, and provide client with a specific person‘s name and contact information at the referral agency. HOP Intake appointments are scheduled as walk-ins only, an intake will take approximately 1 hour to complete, and must be completed every calendar year.

Can I get an ID voucher?

HOP does not provide vouchers for ID’s or birth certificates directly to the client. To be assisted with obtaining an ID or documents needed for an ID, an individual must be a registered patient of FNCH, be established with a FNCH primary care provider, and have completed a HOP Intake.

A HOP Case Manager will determine whether or not a client is eligible for ID assistance during the HOP Intake or follow-up case management appointment. Clients eligible for an ID will be assisted by a HOP Case Manager in retrieving and paying for necessary documents.

Do you have housing?

HOP does not have a shelter and does not issue housing vouchers directly. HOP offers housing referrals/placement into programs that provide shelter and/or housing assistance such as the Barrett House, AOC, the New Mexico Coalition to End Homelessness, Albuquerque Heading Home, Navajo Housing Authority, Albuquerque Housing Authority, Subsidized Housing, Low Income Housing, etc. HOP Case Managers can complete VI-SPDAT Coordinated Entry housing assessments and individuals who have income can be assisted by HOP Case Managers in looking and applying for affordable housing options.

Can I get a motel voucher?

HOP does not provider motel vouchers at this time.

Do you provide rental or utility assistance?

Rental and utility assistance are approved on a case by case basis by the FNCH CEO. If a client is approved for assistance they cannot be assisted with an amount more than $100. If a client owes more than $100 they must pay a partial amount of their bill to lower the balance to $100 or less before assistance is provided. Clients must be able to continue paying their rent or utilities if they are assisted. If clients cannot continue to pay their rent or utilities, they are not eligible for assistance.

In order to be eligible for rental or utility assistance, all persons listed on the lease, including children, must register as a FNCH patient and establish with a FNCH medical provider. All adults (18 years or older) must also complete a HOP Intake.

Required documents for all persons listed on the lease:

  • FNCH Homeless Outreach Letter
  • ID
  • Certificate of Indian Blood (CIB), if applicable
  • Proof of Income
  • Lease agreement
  • Complete the rental assistance application
  • Past due rent notice or eviction notice (must be issued within same month of request)
  • Utility bill (must be issued within same month of request)
