At First Nations Community HealthSource, our Diabetes Program is dedicated to managing and preventing diabetes through a comprehensive team approach. Our team consists of medical providers, including a Public Health Nurse, who offer personalized diabetes prevention education. In addition to individual education, our staff conducts home visits and community-based education at local schools, health fairs, and community events. Our staff is committed to incorporating evidence-based practices to ensure that our program is effective and responsive to the diverse needs of our patients.
